 I ___, agree with the following statements.

1) I have read and understood this office's Privacy Policy.

2) I understand that I may come in contact with confidential information during the course of the performance of my duties and hereby undertake to keep in strict confidence any personal and sensitive information in accordance with the office's privacy policy and applicable laws, including those that require mandatory reporting.

3) I will not divulge any confidential information that may be gathered about the client.

4) I also agree to never remove any confidential material of any kind from the premises of this office unless authorized as part of my duties, or with the express permission or direction to do so by lawful authorities.
